    Church Facility Use Request Form

    Please make sure that this form along with payment reaches the church office at least four (4) weeks prior to your event. If you have any questions, please feel free to contact our church office at 903.347.5110.

    Cost of Facility Use

    The cost of the church facility helps Frost Baptist Church with the maintenance and utilities for the building.  To secure your reservation please submit this form and return your $250 building deposit plus $250 security deposit four (4) weeks prior to your event. 
    ​The security deposit will be returned if building, grounds, and equipment are left clean and undamaged.

    Rules for Facility Use

    We ask that you and your group remember that this building is a church at all times. 
    ​
    We are glad to help with facilitating your event but please consider the following rules are to help protect the reputation and ministry of our church in the community.
    1. No alcohol, smoking, drugs, or dancing will be allowed in the building or on the church grounds.
    2. Do not use course, foul, or inappropriate language while on the church premises.
    3. Please make sure all music playing is family appropriate.
    4. Contact the church immediately if anything is damaged
    5. Make sure a representative from the church is present in order to lock up before leaving
    6. Please follow all guidelines for building and kitchen use.

    Responsibilities after Building Use

    Please note: It is the responsibility of the group or individual using the facility to set up, clean up, and return the facility to normal setup after the approved event or activity.
    1. Collect all garbage into bags and bring it out to the trash cans located by the Stroud St. entrance. (This is located at the front of the building.)
    2. Wipe off tables. If food or drink involved, wipe all tables clean using a mild soap and water solution. If stains occur please notify office so correct cleaning solutions can be used to remove stains.
    3. Return all tables, chairs, stands, and other equipment to their proper places after your event. Return all rooms used to their normal set up.
    4. If using the kitchen, please wash and dry all dishes used and return them to the correct cupboard. Take all extra food and beverage with you unless specific plans for storage are made. Wipe all counters and leave kitchen clean and ready for the next use.
    5. Sweep all floors and mop as needed. Report any damage to equipment or property promptly to the office.
    6. Remove any items put up on the walls or set out in connection with your event.
    7. If the building is not in use when finished. please check that all doors are locked, windows are closed, and lights are off. Then lock up.

    Kitchen Etiquette

    1. Check with the church office for availability of date desired.
    2. There could be more than one activity that uses the kitchen in the same week. Therefore, we request that you check with the church office before you bring your supplies to the kitchen ahead of your scheduled event.
    3. Please return items to designated cupboards. 
    4. Please take home all food and beverage items. We have limited space to store these in our refrigerators.
    5. CLEAN UP: Please wash all dishes, put them away, wipe counter tops and stove with mild soap and water solution, clean sink and sweep floor if needed. Leave used dish cloths and towels in sink. TRASH should be put into bags and taken to the garbage cans located by the front entrance facing Stroud St.

    • ​I also acknowledge that I am responsible for making sure that my group/party follows these rules while the facility is in use.
    • You agree to text messages to confirm reservation, a reminder of reservation, or if there are any changes in time or date, as well as cancellation message in the event you cancel your reservation.
